Liability Release

a. I acknowledge and fully understand that I will be participating in Yoga Classes, personal training and other programs by Stacy Brown, Get Flexy Studio LLC and its instructors during which I will receive information and instruction about yoga, stretching, fitness, nutrition and or health and wellness. I understand Stacy Brown and her staff are not physicians or psychologists, and the scope of their services does not include treatment or diagnosis of specific illnesses or disorders. 
b. I recognize that exercise requires physical exertion, which may be strenuous and may cause physical injury, and I am fully aware of the risks and hazards involved. I agree to assume full responsibilities and assume the risk for any injuries or illnesses, which may occur in class or from services rendered, and waive any claims I may have against Stacy Brown, Get Flexy LLC, or its instructors. I warrant that I am physically fit and have no medical condition which would prevent my full participation in the services offered by Stacy Brown, Get Flexy LLC and its instructors and understand that it is my responsibility to advise the staff of any current physical injuries or health concerns you may have. If you suspect you may have an ailment or illness that may require medical attention, then you are encouraged to consult with a licensed physician without delay. 
c. I waive, release, and discharge Stacy Brown, Get Flexy LLC and its staff from any and all liability, including but not limited to, liability arising from the negligence or fault of the entitites or persons released, for my death, disability, personal injury, property damage, property theft, or actions of any kind which. may hereafter occur to me in traveling to and from this activity. 
d. I agree to give Stacy Brown, Get Flexy LLC and its staff permission to use photographs of myself or my child for any promotional materials. I understand that my child will not be identified by name, nor will any compensation be extended for such use. 
I have read the above release and waiver of liability and fully understand its contents. I voluntarily agree to the terms and conditions stated above.
